
Centre launches mobile app to streamline clinical decision-making by CHOs
The Union Ministry of Health and Family Welfare has launched a mobile application to support Community Health Officers, aiming to strengthen clinical workflows and improve patient care at Sub-Centre Ayushman Arogya Mandirs across India.
The initiative marks a push towards digital health integration at the grassroots level. By equipping frontline providers with structured clinical guidance, the move is expected to enhance the quality, consistency and timeliness of care, a critical step in India’s progress towards Universal Health Coverage.
Developed by the Indian Council of Medical Research, the application serves as a clinical decision-support and job-aid tool aligned with the 12 service packages under Comprehensive Primary Health Care. It provides structured workflows that function as quick-reference checklists for outpatient management at SC-AAMs.
The tool offers step-by-step guidance on patient assessment, including history taking, physical examination and diagnostic testing based on presenting complaints. It enables Community Health Officers to identify cases requiring urgent referral and provides protocols for pre-referral stabilisation.
For non-emergency cases, the application supports decision-making by recommending appropriate management at the facility level, referral pathways or teleconsultation with higher centres. Its design includes three core sections, workflows, assessment tools and treat and counsel.
A colour-coded system simplifies clinical decisions. Red flags indicate immediate threats requiring urgent referral; orange signals the need for specialist evaluation; yellow denotes manageable conditions at the primary level; green represents mild or routine cases.
The application also integrates patient registration, electronic health records, teleconsultation support, diagnostic reporting and follow-up tracking. It is designed to connect with existing national digital health platforms to ensure continuity of care.
Dr. V. K. Paul, Member Health, Niti Aayog, emphasised that primary healthcare forms the foundation of the country’s health system and is central to achieving Universal Health Coverage. He noted that the launch coincides with eight years of the Ayushman Arogya Mandir initiative and provides a practical digital tool for frontline workers.
Dr. Rajiv Bahl, Director General of ICMR and Secretary, Department of Health Research, described the launch as the beginning of strengthened digital clinical support at the primary level. He said the application will evolve through field use and feedback, highlighting the importance of a collaborative and evidence-based approach in building effective digital health solutions.
The new application is expected to empower Community Health Officers with standardised clinical protocols and real-time decision support. As it evolves, it could play a key role in improving service delivery and reinforcing primary healthcare systems across the country.
